your WIN program overview
Eligible SPARC Group families are provided a combined $15,000 lifetime maximum toward the following benefits:
Fertility Benefit: Coverage for certain eligible expenses related to fertility treatment and related fertility medications.
For SPARC Group families enrolled in an Anthem health plan, the Fertility Benefit is integrated with the Anthem medical coverage and all plan cost-shares, including co-pays and deductibles, will apply.
For benefits eligible SPARC Group families not enrolled in an Anthem health plan, please contact WIN for assistance with finding a provider, obtaining treatment authorizations, and understanding any out-of-pocket costs.
Adoption & Surrogacy Benefit: Reimbursement for eligible expenses associated with adoption and surrogacy agreements.

Key highlights of the WIN program include the following:
- Ongoing access to WIN’s Nurse Care Advocates for emotional guidance and support
- Assistance in selecting a high quality Anthem or WIN network provider based on your individual treatment needs
- Expertise in understanding complex information and decisions regarding infertility causes, testing, and treatment option success rates and risks
- Guidance to help increase efficient use of hormonal medications to avoid wastage and the risks of over-stimulation
- Education on your pharmacy dosing usage, storage, and medication side effects and also assistance in maximizing your infertility medication benefit
- Education and advice on adoption and surrogacy processes
- Recommendations or referrals to experienced surrogacy agencies
- Nurse consultations available to expecting parents and those wanting to understand a surrogate’s journey
- Member savings, through the WIN Consumer Program, of 10-40% off medical treatment and pharmacy retail prices for those who have exhausted their benefit, with financing options to make paying for treatment even more manageable
