Eligible City National Bank members enrolled in the Anthem PPO & HDHP plans are provided the following benefits:
Fertility Benefit: A $15,000 lifetime maximum benefit toward certain eligible expenses related to fertility treatment and a separate $15,000 lifetime maximum benefit toward related fertility medications.
Fertility Preservation Benefit: Reimbursement for elective egg and sperm freezing as part of the fertility benefit maximum, including 1 year of storage.
All eligible City National Bank employees are provided the following benefits:
Adoption Benefit: A $10,000 per adoption maximum reimbursement toward eligible expenses associated with adoption agreements, with a $20,000 annual maximum for multiple adoptions.
Surrogacy Benefit: A $30,000 lifetime maximum reimbursement toward eligible expenses associated with surrogacy agreements.
