Missouri Consolidated Health Care Plan members enrolled in Anthem are provided the following benefit:
Fertility Treatment: A lifetime maximum benefit of 2 cycles toward certain eligible expenses related to fertility treatment and fertility medications.
A Diagnosis of Infertility is required to access this benefit.
